Loc-Doc Security Cell Phone Repair and Replacement Plan

1. Overview This document outlines the terms and conditions of the Loc-Doc Security Cell Phone Repair and Replacement Plan ("the Plan"). The Plan is designed to provide employees with cost-effective repair and replacement options for only company-issued cell phones and related accessories.


2. Features of the Plan


a. Screen Repair


  • A screen repair service is available for $50 per incident.

  • A waiver for this cost may be issued at the discretion of the mobile specialist. 

  • A claim must be filed to redeem the screen repair service.


b. Device Replacement


  • If a device is considered to have suffered catastrophic failure, then a device replacement will be necessary. 

  • Catastrophic failure is defined as damage that would exceed more than 60% of the phone's remaining balance. 

  • A device replacement fee of $250 or the remaining balance of the phone, whichever is lower, will be issued. 

  • Employees will earn one (1) waiver of the device replacement fee for every five (5) years of employment.

  • A maximum of two (2) waivers may be banked at a time. 

  • A claim must be filed to redeem the device replacement service.


c. Phone Protection


  • Phone protection, including a protective case and screen protector, is provided for company-issued phones.

  • Phone protection items are covered fully under the Plan and can be requested verbally from the mobile specialist.

  • Please be aware that these items may need to be ordered. 


d. Chargers and Cables


  • A charger and charging cable are provided at the time of hire for company-issued phones.

  • Replacements for chargers and charging cables are available upon verbal request from the mobile specialist.


e. Vehicle Mounts


  • Vehicle mounts for company-issued phones are available upon verbal request to the mobile specialist.

  • Please be aware that these items will need to be ordered and subject to the discretion of the mobile specialist. 


3. Claim Process 


  • The employee must file a claim through the current claim filing method. 

  • The claim process requires submission of relevant details, including, but not limited to, the nature of the damage, how the damage occurred, and when the damage occurred. 

  • Claims will be reviewed, and approval is subject to company discretion.


4. Additional Terms


  • The Plan applies exclusively to company-issued cell phones and accessories.

  • The mobile specialist and the company reserve the right to refuse coverage under the plan due to abuse or misuse of the plan. 

  • The company reserves the right to modify or terminate the Plan at any time with written notice to employees.
